Important Information Regarding Athletic Physicals…PLEASE READ!

The Missouri State High School Activities Association requires a yearly physical examination of all athletes prior to their participation in interscholastic athletes.  The forms for this examination are found on festusathletics.com under the SPORTS FORMS tab.  Medical certificates are valid for the purpose of this rule if issued on or after February 1 of the previous school year.

Coaches are not to practice or play an athlete without the medical form properly filled out and signed by a doctor.  The form also must have all the necessary student and parent signatures and the insurance section completed.  All the necessary forms for a student athlete to receive a practice card from the AD/Middle School office are within this ATHLETIC PACKET.

If a physical is faxed, it must be accompanied with the entire packet and not just the physical section from the doctor’s office.  If every page is not filled out correctly/completely, the form will not be accepted.  Only the AD/Middle School, or an appointed person from the AD/Middle School office, will accept physicals and paperwork that would enable the athlete to acquire a practice card.  Coaches will not accept physicals or paperwork…everything must be turned into the AD/Middle School office.
